Aberdeenshire Council - Social Work (South)

    Social Work

Social Work offers individuals and families advice, practical assistance, counselling and information on services.

    Care Management

  • Care Managers can help people by producing an Assessment of their overall needs. This is done by working together with individuals, their families and carers.

Once an Assessment of Needs is completed care is planned to meet those needs. A copy of the agreed Assessment of Needs Form and the Care Plan is given to the individual.

The Care Manager will make sure that people receive the right services, co-ordinated to suit their needs. Where there are gaps in services the Care Manager makes sure that this information forms part of the future planning process.

  • Community Dementia Team

  • Offers support and counselling for people with dementia and their carers. The support offered is available to anyone requiring information and help.
  • Home Care Service, Community Alarm Scheme, and Meals on Wheels.

  • The above services are managed by the Home Care Team throughout Aberdeenshire South. These three services are available to people of all ages who are permanently or temporarily unable to care for themselves.
  • Home care

  • The Home Care Service is available to people of any age who are permanently or temporarily unable to look after themselves and who wish to continue living at home. Home care will assist with essential Care tasks and complement any existing care, to meet a person’s assessed needs. Home Care is available from early morning to late at night, at weekends and overnight in certain circumstances.
  • Community Alarm Scheme

  • This scheme is available to people who are frail or disabled and at risk and who may require to summon help in an emergency. The alarm system is linked to a control centre where an operator, available 24 hours per day, is able to speak directly to the person and check the assistance required. The operator can telephone nominated contacts or in an emergency call a doctor or ambulance to provide assistance.
  • Meals on Wheels

  • This service is available to people who have difficulty in making their own meals. Meals are delivered hot and ready to eat by volunteers from WRVS. In most areas they are available two days per week.
  • Occupational Therapy

  • Occupational Therapy support is aimed to help individuals enjoy maximum independence and quality of life. This may include:
  • Community Development

  • Community Development Workers work with Voluntary and Community Groups to assist them to develop services in the area of community care.
